INDENTIFICATION

  ● Driver’s license/State ID

  ● Social Security Cards/letter

  ● Birth Certificate (for yourself and your children)

PROOF OF INCOME

  ● TANF,

  ● SSI/SSA, Disability

  ● HUD

  ● Last 2 Check Stubs or Job Letter

HOUSING INFO

  ● Copy of your Lease (If applicable)

THE NEXT STEP

After we receive your application, we will contact you to set up an appointment for a virtual intake assessment. Upon completion of your intake, and return of a complete application, your file will go to the board for approval which can take from 5-7 business days. Feel free to give us a call or contact us by email if you have any questions.
