Below are 5 steps to complete the Board of Director Application. Please complete each step as required.

Step 1: Get your background check.

Request a name background check (not a fingerprint background check) from the Missouri State Highway Patrol.

Visit https://www.machs.mo.gov/MocchWebInterface/home.html and follow the directions for ordering and completing the background check.

Note: Completed responses to criminal record check requests are returned to your account, but responses are not mailed or e-mailed to you. Please log back into your account every couple of hours to check the status.

Download/save the background report and upload it to the upload link below.
Step 2: Complete the Board of Directors Application

Click "Board of Director Application" and complete the application.

Once complete, submit your response. You will receive a copy of your application at the email you list.

We will contact you for an interview, once we receive all of your required documents.
Step 3: Complete the Board of Directors Reference List

Click "Board of Director Reference List" and complete the application. Please note that we will contact them in reference to your application. Please provide accurate information to streamline a speedy process.

Once complete, submit your response.
Step 4: Complete the Board of Directors Narrative Questionnaire

Click "Board of Director Narrative Questionnaire" and complete each question with your best possible answer. Provide and answer to each question using complete sentences and proper grammar.

Once complete, submit your response.
Step 5: Upload required documents to the Upload Center

Click "Board of Director Upload Center" to upload the following required documents.

A copy of your background report. from the Missouri State Highway Patrol dated within 5 days of your application.

A copy of your current driver's license/ID or passport.

A copy of your updated/current resume or curriculum vitae.

A recent and professional headshot of the applicant.

Copies of any certificates, degree, associations, awards, etc. (Optional)
